Patients

We would like to take this opportunity to welcome you to our practice. Please complete the intake forms prior to your appointment date. Please try to be as thorough as possible so that we may make your visit both pleasant and efficient.

We would ask that you come to the office at least 15 minutes prior to your scheduled appointment time, so that we may process your paperwork and clarify any questions you may have. Please note that beginning October 1, 2023, a no-show fee of $50 will be assessed, prior to rescheduling, for appointments missed with no notice. To avoid this fee, please provide 24 hours notice if you will miss your appointment.

Please bring to your appointment:

     A. Completed patient intake forms.

     B. Your referral/ authorization (for HMOs). A hand carried copy is most reliable.

     C. Your insurance card.

     D. Any required co-pay.

     E. Proper identification (i.e. Driver’s license).

     F. A list of your medications, if not already entered on the forms.

     G. All pertinent medical records (X-rays, CT scans, MRIs, pathology reports, etc.).

Please understand that you will not be seen without your referral/authorization and your insurance card. It is your responsibility, as an active participant in your health care, to obtain the proper referral/authorization form. If you are not sure whether your visit will require a referral/authorization, please contact your primary care physician or call our office at (909) 981-5859 or (909)464-1006. The office is quite busy and time constraints will not allow for the office staff to make phone calls to obtain your referral/authorization form on the day of service.

Foothill Ambulatory Surgery Center

The Foothill Ambulatory Surgery Center (FASC) takes great pride in welcoming you. Our skilled team of nurses, technicians and anesthesiologists are committed to making your visit a pleasant and successful experience. FASC is a single specialty ambulatory (outpatient or same day) surgery center for ear, nose and throat and facial surgery. The Centers for Medicare and Medicaid Services (CMS) maintains oversight for compliance with the Medicare health and safety standards. This surgical facility is adjacent to the Inland Ear, Nose and Throat offices, conveniently located near San Antonio Regional Hospital and adjacent medical and diagnostic services.

Billing And Insurance

We participate with most local and many national insurance plans. However, it is your responsibility to understand whether your insurance has limits on the doctors you can see, or the services you can receive. If you provide complete and accurate information about your insurance, we will submit claims to your insurance carrier and receive payments for services. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts. Please contact our billing office at (909) 981-5859 or (909) 464-1006 or call your insurance carrier should you have questions.

Financial

All office visits are payable at the time of service. We accept cash, checks, and most major credit cards and debit cards. Please contact our billing office should you have any financial questions or concerns.

Prescription Renewal

Prescriptions are renewed during normal office hours. In some instances, we may require that the patient be seen in our office prior to medication renewal.

Home Recipes: Nasal Rinse Recipe

  • 1/3 TEASPOON SALT
  • 1/3 TEASPOON BAKING SODA
  • 1/3 TEASPOON WHITE KARO SYRUP (OPTIONAL)
  • 8 OZ WATER
  • 1 BABY SIZED BULB SYRINGE/ 10 CC SYRINGE

MIX ALL THREE INGREDIENTS INTO THE 8 OZ OF WATER, PUT IN BULB OR SYRINGE AND RINSE.
